Student Agreement & Release Waiver

  1. (Payment Policy) All payments are non-refundable. In some instances, payments can be donated or gifted. Please contact hello@evergreenyoganh.com to inquire.

  2. (Acknowledgment & Release) By participating in this event, I acknowledge and understand that all exercise and movement programs involve a risk of injury, and I voluntarily assume a certain risk of injury. By attending this event, I affirm that I am solely responsible for my health and well-being, as well as my decision to engage in any program of physical exercise. I agree to withhold from any activities or movements, which I feel could cause injury to myself. I agree to listen to my body and monitor myself during the entire event. If I experience any pain or discomfort, I will listen to my body, discontinue the activity, and ask for support from the team / teachers. I assume full responsibility for any and all damages, which may incur through my participation.  I agree to practice the utmost safety precautions while I am participating in this event, and will not deviate from the facilitators’ instructions. I release and forever discharge Evergreen Yoga & Meditation LLC, Evergreen Farm LLC, Katherine McDonnell, Michael McDonnell, and all associated entities, persons (including staff, volunteers, contractors, and instructors) and properties from any liability related to my participation in this event.

  3. (Medical Treatment) By participating in this event, I hereby release and forever discharge Evergreen Yoga & Meditation LLC, Evergreen Farm LLC, Katherine McDonnell, Michael McDonnell, and all associated entities, persons (including staff, volunteers, contractors, and instructors) and properties from any claim whatsoever which arises or may hereafter arise on account of any first-aid treatment or other medical services rendered in connection with an emergency during my time at this event.

  4. (Filming & Photography) By participating in this event, I understand that I am entering an area where photography, audio, and video recording may occur. I consent to be photographed and/or filmed for promotional purposes. I release  Evergreen Yoga & Meditation LLC, Evergreen Farm LLC, Katherine McDonnell, Michael McDonnell, and all associated entities, persons (including staff, volunteers, contractors, and instructors) and properties from any liability connected with the taking, recording, or publication and use of these photographs and videos.


    I UNDERSTAND THAT THIS AGREEMENT IS A LEGAL DOCUMENT.  I HAVE READ AND UNDERSTAND THIS AGREEMENT AND ALL OF ITS TERMS. I AM SUBMITTING THIS AGREEMENT VOLUNTARILY AND RECOGNIZE THAT MY SUBMISSION OF THIS AGREEMENT SERVES AS A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW IN THE STATE OF NEW HAMPSHIRE. I HEREBY ASSUME ALL OF THE RISKS ASSOCIATED WITH ACTIVITIES AT THIS EVENT. I HEREBY AGREE TO IRREVOCABLY RELEASE ALL LIABILITY AND RELEASE AND WAIVE ANY CLAIMS THAT I HAVE NOW OR MAY HAVE HEREAFTER AGAINST EVERGREEN YOGA & MEDITATION LLC, EVERGREEN FARM LLC, KATHERINE MCDONNELL, MICHAEL MCDONNELL, AND ALL ASSOCIATED ENTITIES, PERSONS (INCLUDING STAFF, VOLUNTEERS, CONTRACTORS, AND INSTRUCTORS) AND PROPERTIES. BY SIGNING BELOW, I CONFIRM THAT ALL OF THE INFORMATION I HAVE SUBMITTED IS CORRECT. I AGREE TO CONTACT AND UPDATE THE TEAM IMMEDIATELY WHEN ANY OF THESE DETAILS CHANGE.
